claude-code-review.yml only had pull-requests: read, but the prompt runs `gh pr comment`, which needs pull-requests: write (was failing to post). Also enable use_sticky_comment so repeated pushes update one comment instead of spamming. claude.yml was read-only, so a tagged @claude couldn't respond, commit, or open a PR.
